High levels of safety and High Reliability Organizations are not created by rules. They are the result of being a Learning Organization.
The good news: a Learning Organization is not complicated. You just need a structure of only six elements. The bad news: it is very difficult to create a Learning Culture in an organization, because these elements run counter to what people and organizations normally do.
Luckily, although each element is absolutely necessary, they do not need to be established all at the same time. Each element on its own brings benefits. But they will only develop their full potential when all elements are in place and are properly connected in what we call the DEGAS-Circle.
